A Veteran Card – Gold or White covers physiotherapy treatment that is clinically necessary:
- for all conditions, if the patient holds a Veteran Gold Card
- for specific, accepted conditions only, if the patient holds a Veteran White Card
For direct billing to Veteran’s Affairs, please ensure you have a D904 referral from your doctor. You can easily follow the steps in our ‘Quick Referral’ tab. Your doctor can download the referral template, complete the D904 form, and email it to our reception team. Once we receive the referral, we’ll contact you (the patient) to schedule your appointment.
