Medicare Services
If you meet the following criteria, you may be eligible to receive allied health services through Medicare:
- You have at least one chronic medical condition with complex care needs
- You require care from a General Practitioner (GP) and at least two additional health professionals or care providers
- You are managed by your GP under an Enhanced Primary Care Plan (EPC) or Chronic Disease Management Plan (CDM)
Once your GP completes an EPC or CDM plan, you can be referred for up to five allied health services per calendar year. Medicare rebates are available onsite for Physiotherapy and Exercise Physiology consultations, with an additional gap fee.
Type 2 Diabetes Support
If you have Type 2 diabetes, you may also be eligible to receive:
- One Exercise Physiology assessment (with an additional gap fee)
- Eight free group classes, as part of Medicare’s support for chronic conditions.
